How does this happen? I would think that the system would have pressure relief valves and sensors to safeguard against this. I see a lot of lawsuits coming .

My boss lives in North Andover and has been evacuated from his house. I've been texting with him. He was about to cook his kids dinner when his brother-in-law (works at the gas company) called him up and told him to get the hell out of dodge.

Like what Bear44 said, apparently, this was caused by a major mistake that sent high-pressure gas where it shouldn't have gone. It possible that it damaged the low-pressure lines and gas appliances to the point that they'll need repair or replacement all over these 3 towns.

Man ... the infrastructure that allows us to live our cushy lives can be fragile as hell.

Sounds to me like someone hacked into the gas pipeline system. Before computers and the internet (online systems) it was usually someone screwing up but now days all bets are off.
