There is a note on Primary Arms page that says each Fail Zero BCG is independently inspected by Primary Arms before shipping... Take that for what it's worth.
Mine arrived today. Not sure if it's properly staked. Can anyone tell from this pic?
That's what I was thinking. My Colt BCG is real deep. This is not much of an imprint
I agree with this... Mine is too light for my taste as well. It is staked in the proper place, just not enough impact with the hex heads too me.
After another look I don't like this. There is barely even a mark. Here is a better pic. I will get in touch with PA and have them replace it.

The whole purpose of the staking is to keep the bolts from backing out. That staking isn't even contacting the bolt head. Not good to go. That's poor QC.
Mine is lighter than most I own. I need to do the same (contact PA)
