I did actually see a very similar accident back in tech school. I female chucked up a bit in a drill press and the drill chuck key was safety wired to the top of the drill press because people kept loosing it. She forgot to remove the chuck key and turned the press on. The wire wrapped around the end of her finger and snatched it off.
