If your smith is who I think it was, his work has been slipping in the past few years. That chamber does look a but rough.

Pic 2 is bore scope where the lands touch the chamber

Looks a little scratched up

It may just be the pic, but it looks like you have some bulging at the base of the shoulder. That's very bad, is the result of a poorly cut chamber and would cause this exact problem.

I wouldn't try to polish that out myself. I would have it done by a smith. Also might try resizing a piece of new brass but I'm not sure it would help.

Who was the smith ? Hate to hear that he passed.
