I'm trying to visualize how something like this could happen.... What part of the press would allow that kind of penetration without the case being inserted way up in the die?

I can't imagine how a press could have done that. Seems like it would have really bent the finger sideways if it were authentic. That pic looks like it would have bisected heck out of the bone.
