Key words "should not be". All firearms do not meet SAAMI standards. But they generally do. There also have been cases of the cannelure being in the wrong place from the factory. They are usually sold as "blemished" bullets, but some could get missed.

Same load may chamber in a properly throated chamber, but will not chamber in a non throated chamber. Colt throats their chambers, others may not.
