Surrendering C&R license

I will be moving and would like to surrender my license but I can't figure out where to mail my firearms log Book and how to do it

FFL 03 log book doesn't have to be surrendered. You just keep it for 10 years, IIRC.

and how do you surrender the license

For most, it simply lapses without renewal. Moving is a bit of a wrinkle. I suspect, if you simply don't use your license after the move, you are fine, but you should call the ATF to verify.
