You can get them on Amazon from several vendors, they have to be FD-258 forms. There are singles, 3 packs, 25 packs etc.

Just send a copy of form 1 or 4 and RPQ if doing a trust.

I actually ran by a local store. They gave the the ATF finger print cards for free. Now just got to run by probate court and get fingerprints done tomorrow. Already got pictures and forms filed out so I'll be gtg after that.
