Under appreciated and often overlooked prep items:

Tools. Not only things like hammers and bolt cutters but also things like a draw blade. Comes in real handy for making new handles for your axe or shovel. Really you need a manual version of most electric tools such as a hand drill.
 
Funny story, my daughter used to laugh about my semi-prepper stocking of some items. Couple of years back she read the book One Second After, and with the NoKo Nut-job's recent antics, has started a list of necessary items. Some stocking up, but mostly a prioritized list of necessities and where and how to acquire them if SHTF. Plus upgrading weapons and ammo cache.
 
Under appreciated means little talked about or unknown, mostly. So without stating some of the obvious...

In 3rd world dumps the 3 main killers of otherwise healthy people are diarrhea, skin problems, and upper respiratory. Some peroxide, hydrocortisone, pepto bismol (or a cheap knock off), motrin, and cough drops can go a long way. Keeping a med bag filled with these things can be of benefit, but only if it can be taken along.

IMO maxi pads are some of the most useful items available on the super cheap if caught without medical. They make great bandages, or make shift slings.

Soap, dental floss, spoon, cotton balls, Vaseline, alcohol, matches and striker, candles are a few good things to have around if possible.

Protective glasses. Motocross glasses, etc.

Seasonings, pemmican, honey.

Caltrops. Strategically placed, they are a nightmare for an invader.

Plenty more, but enough of this for now.

Oh, and we need God. Certainly under appreciated and often overlooked.
