What’s the worst accidental discharge you have seen!

Don’t have pictures but was near a ND that went through the top of the persons foot and expanded in the sole of his shoe on a concrete floor.
The guy appeared to do everything right from where I was standing about 10 feet away and the round went through his holster as he was holstering a loaded pistol. I Did minor first aid, until a doctor took over, he did not need much. I just took his shoe and sock off and found almost no bleeding I think about that hot room with the smell of blood and sweat almost every time I holster a pistol. After that I always lean my hip out to the side so hopefully the round will miss my leg and foot if I ever ND when holstering .
That was just a minor gunshot to the foot of someone else. I like to think I am not a sissy but that left a lasting memory.

Definitely had his finger on the trigger while holstering. Have seen it several times while instructing. Not possible for a holster to pull the trigger on its own. That’s why we always instruct trainees to take their SWEET time holstering the gun since that is one aspect that does NOT require speed. Usually holstering isn’t done until the threat is eliminated; meaning, relax and take your time now.

Worst ND I witnessed was on the range with another security guy. He was clearing the MK48 when the charging handle slipped out of his hand while pulling it to the rear. I had my ear pros off and was looking down range when the round blasted out and ricocheted off a rock in front of the muzzle. We checked ourselves to make sure we weren’t injured. I took over clearing that day after that incident. I near shat myself since I wasn’t anticipating it and that mofo is LOUD as ****.
 
I made a $250 gun purchase into a $1250 gun purchase. Hang around guns enough and its gonna happen.

I was at a gun show in Cumming Ga the dealer at the table next to me was picking up all the pistols and racking the slides he sent a round through the roof . I was 6 feet away it got very quiet in that room he was promptly escorted out.
 
we were shooting a 3 gun at RBGC years ago, my wife was finishing her run, she had just shot the shotgun stage and the RO gave the commands, shotgun unload, show clear, point at berm and u trigger, she did, THEN, he said Pisto,l draw pistol drop mag, point down range and pull trigger she did, and a 40 cal round went thru the edge of the wooden table and the RO (Skindzier) almost **** himself. (He forgot to say unload and show clear)
She ran a G35 with an Eotech mounted and was deadly accurate with that beast, slow but accurate.
 
These, and the responses to come are the reasons I want my own range one day.
I used to shoot at QuickShot when I lived close to them and they make their bread and butter off Group On which brings in toooons of first timers. Quick Shot was really good at safety briefings but there were soooo many times barrels were pointed in dangerous directions with a loaded chamber. Never witnessed a discharge but the ceiling holes showed there were plenty.
 
Looong ago, a friend was practicing his quick draw with his Frontier scout .22 mag.
Got too quick, still has a 40 grain solid lodged right above his knee.
Front sight caught on the holster, bullet rode his femur down to kneecap.
