Could be a tripple or quad load of a fast burning powder like bullseye or 231.

A squib would bust the barrel, that frame and cylinder is busted, meaning overcharged bullet either re-load or factory.

But it looks like a squib that didn't clear the chamber to me... Not that it didn't clear the barrel but the chamber... Then again maybe you are right cause I don't think there would be room to load another round

There is. A picture in a recent American Handgunner magazine of a revolver barrel cut open sideways with 7 rounds logged in the barrel. It was a six shooter, the shooter actually reloaded and kept shooting before he realized there a problemSo I guess you could have enough squibs stack up back to the cylinder and poof!

Actually that gun belonged to a thug that tried to stick up Chuck Norris one night. As you can clearly see Chuck tore the cylinder off with his bare hands. The guy's asshole looks even worst.
