I do the same. If we follow the line of reasoning that clambering a bullet is waistline precious time then we should also carry in hand as unholstring takes time as well. I think it's important to make the decision early so time for both is available

Probably a round that wasn't properly charged or wet powder. The primer can push the bullet far enough to make this happen.
