Putting your finger inside of an open bolt weapon lime an M249 or M240...even a .50 cal and the bolt released slamming forward (side note: weapons listed above are just examples, but not from your standard rifle bolt). I guess it could've also been from trying to clear a double feed in an AR platform and the bolt release got tapped slamming the bolt forward?
I thought after 16 pages I would have got to the answer!
