Shane Allred at Kyodee Custom in Cedartown has done a few for me (mostly competition style for me, but he does EDC work too.)

I had one made by 4 Corners Concealment. I think they we're based in Augusta or Savannah but I communicated with them by email and they shipped the holster by UPS promptly.
