Trademarking a Name/Slogan

Good Evening,

I'm considering trademarking a Name/Slogan for Business use in the future. Right now it looks like all I need to do is go here:
https://www.uspto.gov/trademarks-application-process/filing-online/initial-application-forms
and fill out the TEAS Regular form.
My questions:
Is this the correct form to trademark a company name/slogan to use in the future? Or do I need to have an existing business with the name already in use/filed in GA.
This name/slogan could potential be used in multiple states, does this form cover that? (I believe so)
Any comments, opinions, suggestions, are much appreciated.

Looks like between $75 & $500

That is a heck of a spread. What is it based on- number of states covered?

It looks like it’s based on type of trademark used according to my link. Those three options range from $200-$400. I believe the $400 option covers you for all 50 and you don’t have to put as much info.

All you need to do to establish trademark is to start using it commercially, with regularity, before someone else in your same field/product category.
