How does this happen? I would think that the system would have pressure relief valves and sensors to safeguard against this. I see a lot of lawsuits coming .

Sounds to me like someone hacked into the gas pipeline system. Before computers and the internet (online systems) it was usually someone screwing up but now days all bets are off.
