Bent shell / FTE issues Keltec PLR16

Need some advice and insight as to what could/typically does cause a shell to bend like this during extraction. I've had a few random FTE's as well, but the gun usually runs really well, until I noticed the shells (ALL OF THEM) coming out of the gun were looking like the one below....

In my experience, the reason for the flat spot on the ejected casing is from the casing being slammed into the receiver upon ejection.
 
Check the upper receiver and brass smudges will typically occur where the shell makes contact. Inspect the extractor for rounding off and if it has a spring take it out and clean it along with the channel. Check the ejector and make sure it's not dinged, bent or otherwise boogered up.

The extractor is a common denominator in weapons malfunctions as is the magazine. Extractors are typically cheap to replace.
