The only thing that I could think of is that either the ball just exploded and the bottom charge didn't go off or the bottom came off the tube? Or maybe it was upside down?

This was my thought as well.

And must've been one of those canister type 500gm loads or something. The little walnut mortars we shot I don't think would take your head off. But what do I know?
