If the date on the county issued license was incorrectly formatted or transcribed and was not noticed by the issuee, then it's not evidence of any evasiveness unless a conspiracy is being alleged.
My critical reading of the report (lines 9-20) does not reveal what answer was given when initially asked. If there were some, ... oh, I don't know... digital recording of his answer maybe, of the first time he was asked and he answered clearly and correctly, then the officer's statements might be called into question. :whistling innocently
eta: OCGA 16-11-24 requires more than non-cooperation in answering questions that one is not legally obligated to give.
The application form does not offer instructions for filling out the DOB field, it simply provides a section seen as "___/___/_____". The license itself also doesnt specify the date format either, showing it in the same XX/XX/XXXX format. So, the "correct" format could be debatable I think. I see it one way, you see it another way, but neither one of us can say the other is mistaken.
For example:

You might read that as "one shoe" or "ones hoe". Without context, it might be either one.
