The squeeze only "cocks" the striker, it is a great design and one of my favorites.

get him a gp100, with a wolf spring in it. i he thinks he can handle the 357, weight helps with the recoil and 38's through it make it feel like a cap gun. can shoot it in single action if he wants as well. otherwise i second a LCR

Mossberg 500 Persuader with the nylon hand loop. Should not interfere with his arthritis. IF the mere sound of just racking the thing doesn't send em running, it will do the trick in any urban Clint Eastwood scenario.
