Form 4 is a transfer of nfa item, so the manufacture puts their engraving on there.
Form 1 is Application to Make and Register a Firearm, therefore the builder needs to engrave their info on it. As the maker of the SBR you become the builder. Therefore you are required for domestically made (SBR) the city and state (or recognized abbreviation), caliber or gauge, and the manufacturer’s name (or recognized abbreviation), where you as the manufacturer made the sbr.
