Question for my first Form 1

I am filling out a form 1 for a SBR that I am putting together and have seen confliction info on the info that goes in
Section 4 block a for the Firearms descrption. I have seen that you put the Manufactures name and location and have also seen put you Trust Name and location. Since this is a lower I am building an SBR off of it would be the Lower Manufacture info in that block correct?
