Maybe the weight of AR-M5F rail and optic allows it to close without being fully seated if you’re not paying attention.

That's a good point.

I just grabbed it out of the safe. I think you might have indirectly solved the problem.

Looks like if you have the tube pushed all the way forward, it will still latch but not be completely level when locked in.
This is what that looks like.

If I put it back in and slide it to the back, there is an audible click and seems more secure as shown here

That very well could have been the issue in the video.

That seems like a likely candidate, the top hand guard definitely shouldn’t be jumping

is the piston on the carrier WELDED on? or just pinned & can tilt separate from the carrier?

That is a lot of play , that gas tube is not locked properly and if it is , then that tube is too short
