I would agree if it wasn't a revolver usually a squib in a revolver you would have two bullets now stuck n the barrel.

My guess its a timming problem.

Maybe the timing was way off?

If that was the case then the firing pin would be so far off center NOT making contact with the primer the bullet would not fire, a slight timing problem would cause damage to the outside edge of the barrel from the bullet shaving as it left the cylinder even though it was detonated slightly off center with the firing pin still making contact with the primer.
