This is more of an FYI type of conclusion:

Took the gun to the range today for more analysis. I put the few that misfired in a separate pile, some of them fired on the second attempt. Two of the four misfires, today, were Hornady 300 Blk head stamps, the other two were 5.56 converts. All of the bad brass had shoulders pushed back far enough for me to be able to pick them out of a crowd—I stood them up next brass that did not misfire. So, now, I will be paying more attention.

Somewhere out there is a sizing die/shell holder combination that sets the shoulder back too far. I use a Lee shell holder and either a Hornady or Lee Full Length sizing die, or a neck size only die. My setup will not even touch the shoulders on the misfiring brass-even if I fire it on a second attempt.

I did find a way to blow the shoulder back out: reloaded the cartridge so that the bullet pressed into the rifling as the bolt was closed. This jammed the case against the bolt and the brass was fire formed back to being in spec. -at least usable spec.
