What Kind of "eye protection" will stop a 2.5 inch long steel spear
(firing pin / striker) that has been blown towards your face from an explosion
that generated over 35,000 psi.??

May not have been the gun at fault.
A firearm is a device used to contain an explosion and release the pressure in a controlled way.
That is kind of like having an argument with a red headed wife/girlfriend, it can go bad so easily.
I know this from experience.

I have on several occasions caught people buying or loading the wrong ammo into a firearm.

We need a link to the story.

"what do you mean 9mm mak dont work in a glock"

Do people really overload their powder, more than say a book recommends.. honest question

I never do. Start at 10% under max and will take it to max, but test the "work ups" first. If any sign of over pressure (and it comes on fast), won't go there. There is a reason why manuals are published.

I am wondering if those pistols are rated for +P+?

This shows one reason I might be leary of loaded chamber indicators ( or really, just a set striker position).

Check out middle picture. Backplate has hole to allow striker to "indicate" setting.

Still wonder if there was an "ammo" issue?
