Can anyone who has filled one out for a trust PM me so I can ask some questions. Just not sure how to correctly fill this out when putting names of myself or the trust on it.

How to Fill out an ATF Form 5330.20 for a Trust - Certification of Compliance with 18 U.S.C. 922(g)(5)(B)
In the past the ATF has not required a Form 5330.20 - Certification of Compliance with 18 U.S.C. 922(g)(5)(B) for a trust because a "Trust" does not have a nationality, but lately, several individuals at ATF have started requesting them, and denying applications where they were filled out incorrectly. To resolve this issue we are creating this page to help trustees understand how ATF would like them filled out.
1. Name under which the application was filed. (Last Name, First Name, Middle Initial - or - Name of Corporation, Partnership, or Association, if applicable).
If you are making application under a trust, put the trust's name here
2. Name of person certifying to citizenship. (Last Name, First Name, Middle Initial)
This should be your Last Name, First Name, and Middle Initial as stated
3. What is your COUNTRY(ies) of citizenship? (List additional countries if applicable)
UNITED STATES OF AMERICA
For Non citizens you will need to fill out the questions 4-6 based upon your actual information
US citizens you should answer NO for Question 5 and sign your name and date the form in box 7
Whether your agent requires it or not, it is a simple form and to avoid delay we would now recommend that you include it with your paperwork.
It depends on the examiner. I've heard some don't want them and some do. To be safe I send them in with ALL form 4s.
no you send it with your form 4. I've never sent one with mine before, but I've heard it's a new thing.Now, is this another form they want you to fill out after the fact? I mean, is this compliance for something you already have?
