Transferring Form 1 SBR to trust questions

I'm wondering if anyone could share some insight on a few questions I have concerning transferring my individual SBR's to my new trust. I've emailed ATF both through the 'ask the experts' on eforms and NFA@atf.gov as far back as a few weeks ago and have yet to get a response. I've also called and talked to a lady there who, while very nice, didn't have an answer and had to transfer me to her supervisor who hasn't called me back. Anyway, here are the questions I emailed them.

Hi,
I currently have two SBR's registered to me as an individual on Form 1's since 2006. I have recently set up a NFA Trust and would like to transfer these SBR's to the trust via Form 4's. In 2009 I moved to a new in-state address one county over and believe I should first submit a Form 20 to update the registry before submitting the Form 4's.
If this is correct, could you please clarify a few questions before I mail the forms?

a) On the Form 20, what dates should I
list as 'in transit' since already at new
address?
b) Should I include a cover letter with the
Form 20 requesting that response be
sent to new address?
c) Should I wait until the Form 20
'change of address notification' is
approved before mailing the Form 4
applications?
d) On the Form 4, in block 3a, should I
list the new address since I would
have submitted a Form 20
beforehand or list the the original
address as it appears on the Form 1?
e) On the Form 4, in block 4a, do I list
myself or the original manufacturer
since it asks for 'maker, manufacturer,
and/or importer'? I figure I would list
my name, city, and state in block 4h
of the Form 4 as it is engraved on the
receivers per the original Form 1,
block 4h requirement?
