I don't, I always make sure it's in full contact with my forehead for stability. It was really hard keeping the blood out of my eyes until the scar tissue built up.

I'm guessing the rail and thus the screws are second hand? I've never heard heard anything but good things about them. Not that they couldn't have sent out a bad batch of screws though.
I was gonna ask some questions about this as well being that I just installed one of these rails at the start of rifle season but I'm still stuck on the 24 pairs of pants thing
It came with the rifle, so who knows what happened before I got it?

Yeah, that's probably the problem.
The problem is a lot of the cheaper rails don't incorporate a recoil lug and the recoil from heavy guns can wear them out
