Likely a combination of factors but it's an incredible, catastrophic failure. You'd think someone would have noticed something, somewhere, somehow, sometime over the past few years that would have alerted an engineer that there is a problem. I used to work with some structural engineers that became software programmers and their conversations were very interesting whenever we were someplace they could observe large, concrete structures (they used to design stadiums in England).Depends would be full free falling 12 stories with that mess.
