The damage to your hearing is cumulative. Every shot you take without protection is more permanent damage to your hearing that will never come back. If you are flinching in a gun battle, you have more problems than hearing. The line about " I need to know what to expect" makes me smile. You don't go out and have someone shoot you so that you know what to expect. Trust me it hurts to get shot or to shoot without hearing protection. When I was in my 20's, I was 10 feet tall and bullet proof. Now I'm in my 60's and I find that I'm only 6 feet tall and all beat up from the stuff I did in my 20's.

I only do it once a year, New Years eve. Usually only two shots at most, since it will make my ears ring for a day afterwards.
I think I've lost more hearing from concerts than shooting my guns though!

No, and wouldn't ever do it purposely, like someone said the damage is cumulative and tinnitus resulting from hearing loss can drive people crazy. I did however, shoot deer using a .308 and .300 magnum (surely that damaged my hearing) but I don't recall ever having ringing or other effects from shooting at live game. I also have to undergo a hearing evaluation each year with my job and my hearing is fine. I want to keep it that way!

No and you don't need to. In a real world situation you will not notice the muzzle blast at all. Hunters never notice the sound or the recoil of the weapon and the adrenalin pumping in a hunting situation is far less that it is in a life and death situation.

All you would be doing is needlessly damaging your hearing.
