How expensive is having small fabrication done?

Say I had a wooden version of something that I wanted to have fabricated with lower end steel. Measurements are such that nothing is exact, as I'm wanting bolt slots that allow for exact adjustment for me at a later time.

So basically, it would be a rigid piece of rectangular metal with 7 slots cut so bolts can be secured through it. Size is approximately 14" long by 8" wide.

Also, anyone know of someone who could do this sort of thing?

Depending on thickness of the material, it may be a job for a shop that has a plasma cutter.
